Fasting Laboratory Tests

Pre-program laboratory tests must include the following if performed at your private physician's office or lab:

• Sodium

• SGOT

• Potassium

• SGPT

• Chloride

• Alkaline Phosphatase

• CO2

• Bilirubin - Total Direct & Indirect

• BUN

• Cholesterol - Total & HDL

• Creatinine

• Triglycerides

• Glucose

• TSH

• Uric Acid

• Complete Blood Count (CBC)

• Calcium

• Total Protein

• Phosphorus

• Albumin

For patients with diabetes:

• Hemoglobin A1C

• Urine microalbumin or urinalysis

Results must include a list of the normal range values from the lab performing the tests.
